PCP/Specialist/Ancillary Claim Submission Options

You may submit PCP claims by mail, fax, or EDI.

Facility Claims Information

To submit your facility claims through your Clearinghouse, use the Verda Payer ID: VERDA1.

Referral/Authorization Steps

  • For Primary Care Providers (PCPs) Submitting Through Portal
  • PCPs are encouraged to submit authorizations electronically via the PORTERO Portal.
  • To request access:

1. Complete the Letter of Agreement for Data Exchange Form.

2. Email the completed form to:  Trang.Nguyen@Nutexhealth.com.

3. Once processed, login credentials for the PORTERO Portal will be issued. Training can also be scheduled upon provider request.

  • For Specialists (Fax Only Option) or PCPs Preferring Paper/Fax Submission:
  • Use the official HPIPA Referral Form 2025-02 Verda, ensuring all required clinical documentation is included.
  • Fax the completed form to 866‑458‑7482 or 562‑394‑0641.
  • All forms and MSO contact numbers are available under the Resources tab of  our website.
